Consultation






Now let’s setup a doctor consultation so that we can process and ship that order (prescriptions can’t be mailed from the pharmacy until you’ve completed your consultation.)

  • Please complete this form providing your full name and date of birth
  • You will receive an email from Perfect Health Technologies to create an account. You will also receive an additional email with the patient consent forms.
  • Once your account is created, click on “Appointments” tab to request a preferred date/time for your consult. (nutrition consultations are also available but not mandatory). Our doctors work Monday–Friday 9-6 pm and Saturday 8-1 pm PST.
  • Once you’ve completed your consultation with one of our doctors, we’ll submit your prescription to the pharmacy to be filled. Prescriptions are sent via FedEx 3-day shipping. Tracking details will be emailed to you directly.
